Thanks for completing the application to start a new Walk Chapter in your community!

Please read the following notes prior to starting the application:

1. The application can take 10-15 minutes to complete, so please ensure you have enough time.

2. Make sure you click "next" at the bottom until you finally see a summary page with a button at the bottom that says "submit".

3. The form will ask you for information about your organization, points of contact, and walk schedule/location. We can always adjust the information later on, so feel free to write "TBD" or make your best guess.

4. As a reminder, a "chapter" is typically defined by the walk location (one outdoor location and one optional inclement weather location) that you designate on this form. Email us if you need to use additional locations for your program, and we'll talk through some options.
