Waiver Instructions for Walk Leaders
All participants and leaders should sign the waiver before or at their first walk, and then again once per year. We recommend that you give walkers two options for signing at the walk:
Option 1: Electronically
1. Print the e-waiver and write your chapter number in the box if it doesn’t appear automatically.
2. Participants will scan the QR code and complete the information on their smartphone.
Option 2: On Paper
1. Print a copy of the paper waiver and sign-up sheet (several copies if you expect more than 15 walkers).
2. Participants will read the waiver and then sign their name on the sign-up sheet.
3. Submit these sheets when completing your post-event survey after the walk.
