Waiver Instructions for Walk Leaders
All participants and leaders should sign the waiver before or at their first walk, and then again once per year. They have two options for signing at the walk:
Option 1: Electronically
1. Print the e-waiver and fill in your chapter number if it doesn’t appear automatically.
2. Participants will scan the QR code and complete the information on their smartphone.
Option 2: On Paper
1. Print a copy of the paper waiver and sign-up sheet (several copies if you expect more than 15 walkers).
2. Participants will read the waiver and then sign their name on the sign-up sheet.
3. Email these sheets to contact@walkwithadoc.org after your walk.
